2006-2007 Kindergarten Pre-Registration

Pre-Registration of children eligible for kindergarten September 2006 will be conducted in all Mt. Diablo U.S.D. elementary schools on Tuesday, May 2, 2006. Hours of registration are 9:00-11:00 am and 1:00-3:00 pm.

Children who will reach the age of five years on or before December 2, 2006, are eligible for kindergarten in September.

Parents should bring the following documents with them:

A birth certificate or some other official documentary proof of the child's birth date.
A document that indicates where the child lives.
A written immunization record from a health care provider indicating that all immunization requirements have been met. (see below for requirements)

Children starting Kindergarten need a health examination dated March 2006 or later

IMMUNIZATION REQUIREMENTS

POLIO--4 doses are required, 3 are enough if at least 1 was given on or after the 4th birthday
DIPTHERIA-TETANUS-PERTUSSIS--5 doses are required, 4 are enough if at least one was given on or after the 4th birthday
HEPATITUS B--3 doses are required
VARICELLA (Chickenpox) 1 dose is required. Dr. documented varicella disease history or immunity will also meet the requirement

Parents are urged to register at this time to assist school personnel in making plans for the coming year. If you are unsure of which school to attend, please call the District Office (925) 682-8000 or access the district website at www.mdusd.k12.ca.us

DISTINGUISHED SCHOOL CEREMONY AND CELEBRATION

Ayers Elementary School received the honor of "Distinguished School". The awards ceremony was held at the Disneyland Hotel in Anaheim, California.
